Spots will open a telnet window like this, shown as just logged in to GB7BAA:

To respond to the login sequence you will need to set your call in the YOU! button. A right click on the buttons will bring up the button editor:

You can edit these to taste but Button 10 text (long box) will always be sent in response to login: or call: or similar sequences from the spot server. Button 10 label does not have to be YOU! it can be anything.

Choose a server in the 'Cluster:' combobox and a click on CONN will now log you on, unless the server has a less common sequence. Any extra text can be typed in the 'Type:' box from now on.

A right click in the telnet window will show the context menu you can see in the first picture. This has six options:
Hide Spots which will toggle hide/show spots in the span display.
Autostart will attempt to log you on as soon as you click Spots in the menu.
Spot Timeout is the usual timeout for how long spots should remain visible. Setting zero will clear the spots as in clear them, not hide them.
Stack Depth is how many spot labels deep should show on the display to reduce overlaps. On a quiet day this does not need to be large. In CW WPX it needs a large number and a wide display!
Edit Cluster List if there are not enough in the combo box...
Change Font does something.

Once set up and running you will see something like this in the display (part of CW WPX 2014).
